Pulmonary hypertension is high blood pressure in blood vessels supplying the lungs, causing dyspnea, fatigue, syncope, angina, edema, and if chronic, right-sided heart enlargement and ventricular dysfunction. PH is classified by cause: pulmonary arterial hypertension (PAH, Group 1), left-sided heart disease (Group 2), lung disease (Group 3), chronic thromboembolic pulmonary hypertension (CTEPH, Group 4), and PH caused by other conditions (Group 5). PAH therapy is the most developed, with ERA, PDE5i/sGC stimulator, Activin signaling inhibitor, and prostacyclin-based therapies available and used in combination in many PAH patients. Some CTEPH patients can be treated surgically, and Adempas is approved for persistent/inoperable CTEPH. Tyvaso became the only drug approved for PH-ILD (a subset of PH Group 3) in 2021, and no therapies are approved for PH Groups 2 or 5 or PH-COPD (major Group 3 subset). PAH has the largest drug pipeline, focusing on novel (particularly anti-proliferative) pathways, and increasing tolerability/ease of use for established MOAs. Drug development for Group 2 and 3 PH has been increasing dramatically in the past 5 years, particularly for PH-ILD following the approval of Tyvaso.
