Type 1 Diabetes occurs when the pancreas produces little or no insulin, preventing regulation of blood glucose. The exact cause is unknown, but T1D occurs when the body’s own immune system destroys insulin-producing pancreas beta-cells. T1D is usually diagnosed in childhood or adolescence, but also develops in adults. Despite considerable research, T1D has no cure and blood glucose levels are managed via regular insulin administration. Despite novel insulin therapies and insulin delivery systems, glucose control remains suboptimal in most patients. Non-insulin adjuncts can facilitate lower insulin doses to reduce negative effects of insulin (e.g. weight gain), but success has been limited. Recently SGLT-2 inhibitors were approved in Europe, but DKA risk has blocked US approval. Pramlintide remains the only T1D adjunct to insulin approved in the US. Treatments that delay T1D onset are also being explored but the safety bar is high for this patient population.
